SOAP notes and exercise prescriptions per session
Presenting with 6-week right shoulder pain. Overhead activity at work (construction). No acute injury. Pain rated 5/10 at rest, 8/10 overhead. VHI Plan B.
Reduced external rotation R vs L (35 vs 55 deg). Painful arc 80-120 degrees. Neer and Hawkins tests positive. Full cervical ROM. No neurological signs.
Rotator cuff tendinopathy - supraspinatus predominant. Contributing factors: load management and posture at work.
Manual therapy x2 session. Home programme: pendulum exercises, external rotation band work. Advised on posture at work. Review in 1 week.
Week 4 of treatment for IT band syndrome. Pain improving - now 3/10 at 5km, was 7/10 at 2km. Running 3x per week as prescribed. Marathon in 6 months.
Hip abductor strength improving - single-leg squat quality much better. Ober test still slightly positive but reduced. Foam roll compliance good.
Good progress. Tissue responding to load management and hip strengthening. On track for marathon.
Continue current programme. Progress to 8km long run this week. Add nordic hamstring curl. Session in 2 weeks.
Week 24. ACL protocol complete. No pain. Confident. Playing for Greystones RFC. Clearance session today.
Single leg squat: symmetrical. Hop tests: triple hop 98% limb symmetry. Side hop: 97%. 10m sprint bilateral: 1.62s. Running gait: normal.
All return-to-sport criteria met. Quad strength symmetry 94% (threshold 90%). Ready for return to full contact training.
Signed clearance letter issued to club. Gradual return: contact training week 1, full match week 2-3. Monthly self-monitoring. Discharge letter to Mr. Flanagan (Beacon).
Week 5 for bilateral plantar fasciitis. Significant improvement. Morning pain now 2/10 (was 7/10). Running 20km per week. Sprint triathlon in 6 weeks.
Single leg heel raise: 25 reps each side without pain. Windlass test negative. Step test pain free.
Excellent response. Near full resolution. Footwear compliance key factor.
Progress to plyometric loading next session. Cleared for brick training (bike-run). Final review in 3 weeks pre-race.
